Kurt Cobain signed these medical records from his April 15, 1991 visit to the emergency room at St. Peter Hospital in Olympia, Washington in black ballpoint pen. The one page outpatient/emergency record states that he came in for gastric pain and gives his personal information, including address, listing his occupation as "Musician" and his employer as "David Geffen". The documents are dated only days after Kurt signed a record deal with Geffen Records. The second page is signed by the doctor who examined him and gives the diagnosis and treatment. Very good condition. Comes with a Gotta Have Rock & Roll™ Certificate of Authenticity.
